Please only complete this form once you have a plan for funding the program, either through your organization's budget or through an agreement with an outside sponsor. If you are self-paying, the program fee is due two weeks after enrollment. If you are seeking a corporate sponsor, please fill out our sponsor match form instead of this enrollment form. We may be able to refer you to one of our corporate sponsors.
Below, you will be asked a series of questions about yourself and your business. It is recommended that you complete the form in one session or compose your answers in another program and copy them into the form when you are ready to submit. Unless the programs reach capacity earlier, enrollment for the December session of Building a Successful Business will close on October 28. For the April session, enrollment will close on February 24.
